DescriptionWe are seeking a skilled Mental Health Therapist to join our team in Centralia, WA. As a Youth Outpatient Mental Health Therapist, you will provide community-based treatment to youth and their families, working closely with our multidisciplinary team to deliver whole-person, integrated care.
Responsibilities:
- Assess participants' physical and mental status, daily living skills, coping patterns, strengths, and supports.
- Deliver medically necessary care coordination services to CIHS clients.
- Develop and implement evidence-based treatment plans and documentation for data collection and outcomes.
- Provide data and medical history to accelerate successful referral to psychiatric services.
- Ensure annual age-appropriate behavioral health screenings using evidence-based tools.
- Transfer patients to long-term counselors as needed.
- Support referrals and bi-directional tracking with care teams.
- Clinical paperwork, including documentation of all contacts, problems, interventions, diagnostic eligibility, and dispositions.
Requirements:
- Mental Health Professionals with a Master's Degree in counseling, social services, or a closely related field and two years of experience providing direct service to those with mental illness.
- Licensed Agency Affiliated Counselor Certification with the Department of Health (DOH) or ability to obtain within 30 days of hire.
- Positive, solutions-oriented clinicians.
- Therapists skilled at engaging and motivating youth to achieve participation in services.
- Valid driver's license and current automobile insurance, reliable transportation with an acceptable driving record.
- Professional licensure as a LMHC/A, LMFT/A, or LICSW/A is preferred.
