About the Role

The Prevention and Diversion Case Manager plays a vital role in supporting individuals and families at risk of homelessness. This position involves providing direct, time-limited services to participants, including comprehensive assessments of their needs and connecting them with necessary resources such as legal assistance, housing options, and mental health support.

Responsibilities
  • Coordinate and manage all prevention and diversion referrals to ensure timely and accurate service delivery.
  • Assess and guide households in exploring alternative stable housing options to divert them from emergency shelter.
  • Review and respond to referrals from households experiencing or at risk of homelessness.
  • Develop, maintain, and share a comprehensive resource library and related communication materials.
  • Promote program awareness and build trust with community partners to enhance engagement.
  • Provide case management, linking participants to resources such as shelter, permanent housing, health/mental health services, public benefits, and employment opportunities.
  • Conduct assessments and make referrals to support participants' goals and needs.
  • Create individualized, housing-first service plans and assist participants through the eviction process, including housing location services, referrals, and advocacy.
  • Assist with rental assistance packages from various funding sources, properly documenting fund distribution, and leading the recertification process as needed.
  • Address safety concerns quickly and compassionately.
  • Complete and submit all required incident documentation before the end of each shift.
  • Assist participants in securing basic needs such as clothing, employment, and childcare.
  • Empower participants to successfully achieve their identified goals.
  • Provide accessible services to participants, including conducting field visits.
Requirements
  • A bachelor's degree in human services/related field or commensurate experience.
  • 2+ years of street outreach/case management/direct services/other related experience.
  • Strong written and oral communication skills.
  • Ability to prioritize competing priorities and make sound judgments.
  • Ability to complete tasks while navigating frequent interruptions.
  • Ability to deliver creative, resourceful solutions to unique challenges.
  • Strong organizational and time management skills.
  • Valid Driver's License and reliable transportation.
  • Ability to Drive a 15-passenger van.
  • Willingness to work evenings and weekends when needed.
