Key Responsibilities- Case Management: Provide case management services to program clients, ensuring their needs are met and they receive the necessary care.
- Quality Assurance: Monitor and ensure the quality and appropriateness of services provided, adhering to agency and contract guidelines.
- Documentation: Maintain accurate and complete documentation in accordance with agency guidelines.
- Education and Community Outreach: Develop and provide individual and group education for patients and site staff, establishing and maintaining community relationships to support client referrals.
- Infection Control and Safety: Adhere to agency infection control and safety policies, ensuring a safe environment for patients and staff.
- RN Licensure: Current RN licensure for New Jersey.
- Experience: 2+ years of community health nursing experience, with bilingual skills preferred.
- Computer Skills: Strong computer skills, with excellent verbal and written communication skills.
- Interpersonal Skills: Excellent interpersonal skills, with the ability to communicate effectively with outside personnel.
- Travel: Ability to travel with a valid NJ driver's license.
