Independent Optometrist Opportunity

As an independent optometrist, you will have the autonomy to establish your own practice within a Sam's Club location. This opportunity allows you to take a leading role in patient care, providing eye and vision care as well as general health and well-being services.

Key Responsibilities:
  • Establish your own days of practice and hours of operation
  • Control your patient schedule and set your fee schedule
  • Keep 100% of exam/professional fees
Benefits:

Requirements:
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
