- Help patients with mobility and transfer assistance
- Provide oral hygiene and nail care
- Assist with medication management and administration
- Help patients with light housekeeping and laundry
- Prepare meals and snacks for patients
- Assist with grocery shopping and errands
- Communicate with patients, families, and healthcare team members
- Document patient care and progress
- Participate in team meetings and training sessions
- High school diploma or equivalent required
- Current CNA certification required
- 1-2 years of experience in home health or hospice care preferred
- Ability to read, write, and speak English fluently
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Reliable transportation and ability to travel to patient homes
