Job Summary

As a Hospice Care Assistant with Hospice of Michigan, you will provide essential personal care and homemaking services to patients in their homes. Key Responsibilities

  • Assist patients with daily living activities, such as bathing, dressing, and grooming
  • Help patients with mobility and transfer assistance

Assist with Household Chores
  • Help patients with light housekeeping and laundry
  • Prepare meals and snacks for patients
  • Assist with grocery shopping and errands
Communicate Effectively
  • Communicate with patients, families, and healthcare team members
  • Document patient care and progress
  • Participate in team meetings and training sessions
Meet Qualifications
  • High school diploma or equivalent required
  • Current CNA certification required
  • 1-2 years of experience in home health or hospice care preferred
Requirements
  • Ability to read, write, and speak English fluently
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Reliable transportation and ability to travel to patient homes
