About the Role:
As an MDS Coordinator at Napa Post Acute, your primary responsibilities will include:
• Conducting comprehensive MDS assessments in compliance with state and federal regulations
• Developing and updating individualized care plans based on thorough assessments and interdisciplinary team input
• Ensuring accurate and timely documentation to support optimal reimbursement and compliance
• Collaborating with nursing staff, therapists, and other healthcare professionals to deliver exceptional resident care
• Participating in care plan meetings and quality improvement initiatives to enhance resident outcomes
• Monitoring MDS processes to ensure consistency, accuracy, and adherence to all regulatory requirements

Key Qualifications:
To be successful in this role, you'll need to have:
• A current California LVN or RN license
• Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting
• In-depth knowledge of MDS 3.0, care planning, and regulatory guidelines
• Strong organizational skills with a keen attention to detail
• Excellent communication and teamwork abilities
• Commitment to providing high-quality, resident-centered care
