Job Summary:

We are seeking a skilled Occupational Therapist to join our home healthcare team at Western CT Home Care Inc. As an Occupational Therapist, you will work with patients in the comfort of their own homes, providing services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.

You will assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver. Your role will involve communicating with the physician responsible for the health plan of care and other healthcare practitioners as appropriate.

Responsibilities:

  • Develop, implement, and evaluate the plan of care in partnership with the patient, representative (if any), caregiver(s), and physician(s).
  • Provide services that are ordered by the physician as indicated in the plan of care, communicating with all physicians involved in the plan of care and other healthcare practitioners as appropriate.
  • Provide direct occupational therapy services in accordance with the plan of care established by the physician.
  • Maintain records on visits to clients, including evaluation, plans for care, client's response to therapy, and discharge planning when applicable.
  • Participate in case management conferences with the nurse and other team members to coordinate the occupational therapy program with the total plan of care.
  • Provide education and counseling to the patient and family members regarding occupational therapy treatment regimen.
  • Identify patient and family needs for other home health services and make client referrals for same, when needed.
  • Participate in the agency quality assurance and performance improvement program and the agency-sponsored inservice training.
  • Supervise care provided by the COTA(s) and home health aides as appropriate.
  • Re-evaluate the client's progress and treatment regime, making changes as indicated, and under the physician's recommendations.
  • Fulfill all compliance responsibilities related to the position, including in-service training.
  • Perform other duties as assigned.

Requirements:

  • One year of experience as an Occupational Therapist.
  • Desired: Experience in a qualified home health agency.

Working conditions:

  • Significant manual skills/motor coordination and finger dexterity.
  • Very Heavy effort. May exert up to 50 lbs. force.
  • Some occupational risk.
  • Some exposure to dirt, odors, noise, human waste, etc.
