Senior Primary Care Physician

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a key member of our integrated care team, you will provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
About the Role
- Provide personalized, patient-centered care to seniors in their homes, focusing on whole-person health and addressing physical, emotional, and social wellness.
- Work collaboratively with our multidisciplinary team, including physicians, nurses, and behavioral health specialists, to deliver comprehensive care.
- Conduct thorough assessments, develop and implement treatment plans, and provide education to patients and their families.
- Prescribe medication, order diagnostic tests, and refer patients to specialists as needed.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in senior primary care.
Requirements
- Active, unrestricted state license and DEA license.
- Board certification as a Nurse Practitioner.
- Minimum 2 years of clinical experience in primary care or a related field.
- Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare software.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
