Job Summary

We are seeking a Physical Therapist to join our team. As a Physical Therapist, you will conduct initial evaluations, assess patient conditions, and develop appropriate goals and treatment plans.

Key Responsibilities:
  • Evaluate physical and functional status, as well as cognitive, psychological, and perceptual ability to determine the patient's capabilities to function safely.
  • Document all evaluations, treatment, and coordination of care contacts via appropriate facility policies.
  • Demonstrate an ability to treat a wide range of diagnoses and be knowledgeable in current therapy treatments, goals, and contraindications for treatment.
  • Provide education and counseling to patient/family regarding treatments and goals, potential safety risks and/or other issues within scope of practice.
  • Independently manage own case load to maximize effectiveness of overall care plan.
  • Comply with all applicable Federal, State, and local laws, regulations, and requirements, as well as facility/agency specific policies and procedures.
Requirements:
  • Bachelor's or higher degree in Physical Therapy from an institution accredited by Commission on Accreditation in Physical Therapy Education.
  • Current State Physical Therapist License.
  • Current CPR certification.
