MTM Pharmacist Job Description

We are seeking a highly skilled MTM Pharmacist to join our team at HealthCare Support. As a key member of our Clinical Engagement Services Center, you will play a critical role in engaging members telephonically and scheduling pharmacist consultation appointments.

Daily Responsibilities:
  • Engage members telephonically and schedule pharmacist consultation appointments
  • Prioritize patient confidentiality and adhere to HIPAA regulations
  • Utilize prescription claims data to generate clinical recommendations
  • Assess medication regimens for unnecessary medications, ineffective medications, and adverse drug reactions
  • Provide patient education on medication regimens and disease states
  • Accurately document patient care notes and recommendations
Required Qualifications:
  • Remote work experience with a cross-functional team environment
  • Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
  • Current and unrestricted Pharmacist license in the state of Utah
  • Bachelor's Degree in Pharmacy or PharmD
Preferred Qualifications:
  • PharmD
  • Experience working in retail/community pharmacy and/or PBM
  • Bilingual in medical terminology
  • Additional Pharmacist License in good standing

Pay Details: $65.00-$75.00 an hour
