MTM Pharmacist

HealthCare Support is seeking a highly skilled MTM Pharmacist to join our team. As a key member of our Clinical Engagement Services Center, you will play a critical role in providing medication therapy management services to our clients.

• Engage members telephonically and schedule pharmacist consultation appointments to discuss clinical programs.
• Prioritize patient confidentiality and adhere to HIPAA regulations and organizational policies.
• Utilize prescription claims data to generate clinical recommendations for Medication Therapy Management and Retrospective Drug Utilization Review programs.
• Assess medication regimens for unnecessary medications, ineffective medications, and adverse drug reactions.
• Provide patient education on medication regimens and disease states.
• Accurately document patient care notes and recommendations.

• Remote work experience with a commutable distance to Sandy, Utah.
• Bachelor's Degree in Pharmacy or PharmD.
• Current and unrestricted Pharmacist license in the state of Utah.
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services.
• Experience working remotely/independently within a cross-functional team environment.

• PharmD.
• Current and active Certified Diabetes Care and Education Specialist, Board Certified-Advanced Diabetes Management, ASHP Diabetes Management Certificate, or APhA Patient Centered Diabetes Care Certificate.
• 2+ years of pharmacy work experience in retail/community pharmacy and/or PBM.
• Bilingual in medical terminology in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.
• Additional Pharmacist License in good standing in Alabama, Idaho, Kentucky, Louisiana, Maryland, Minnesota, Oregon, Tennessee, or Vermont.

$65.00-$75.00 an hour.
