Job Title: Behavioral Health Case Manager II

Job Summary:

Elevance Health is seeking a highly skilled Behavioral Health Case Manager II to join our team. As a Behavioral Health Case Manager II, you will be responsible for providing case management services to members with behavioral health and substance abuse needs. This is a remote position that requires strong communication and interpersonal skills.

Key Responsibilities:

  • Conduct assessments to identify individual needs and develop specific care plans to address objectives and goals.
  • Monitor and evaluate the effectiveness of care plans and modify as needed.
  • Coordinate with internal and external resources to meet identified needs of members and collaborate with providers.
  • Serve as a resource to other BH Case Managers.
  • Participate in cross-functional teams and projects.

Requirements:

  • Master's degree in social work, counseling, or a related behavioral health field.
  • Minimum of 3 years of clinical experience in social work counseling with a broad range of experience with complex psychiatric and substance abuse treatment.
  • Current active unrestricted license to practice as a health professional within the scope of licensure in applicable states or territory of the United States.

Preferred Skills:

  • Experience in health coaching and motivational interviewing techniques.
