Role Summary:
We are looking for a dedicated and proficient Licensed Practical Nurse (LPN) to enhance our healthcare team. The successful candidate will be responsible for providing nursing care in alignment with established care plans, advocating for the health and wellness of our patients.
Key Responsibilities:
- Deliver direct patient care, including monitoring vital signs, administering medications, and assisting with daily living activities.
- Contribute to the formulation and adjustment of nursing care plans based on patient assessments and progress.
- Ensure accurate and timely documentation of patient care activities and outcomes.
- Engage in effective communication with patients, families, and healthcare team members to facilitate optimal patient care.
- Support patient education by providing information regarding treatment plans, medications, and health management.
- Comply with all safety, infection control, and health regulations and policies.
Qualifications:
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Preferred prior experience in a clinical environment (e.g., skilled nursing facility, hospital, or home health).
- Basic Life Support (BLS) certification is mandatory.
- Excellent communication and interpersonal skills.
- Able to work effectively in a collaborative team environment.
- Compassionate, patient-focused, and efficient in delivering care.
