Join Our Team as a Clinical Pharmacist
UPMC Chartwell is seeking a dedicated Clinical Pharmacist to enhance our healthcare team. You will also contribute to the education of current and future healthcare professionals and support research initiatives related to pharmaceuticals.
Key Responsibilities:
- Conduct thorough reviews and maintenance of patient medication records, ensuring the identification of allergies, drug interactions, and therapeutic duplications.
- Accurately report any suspected adverse drug reactions in a timely manner.
- Interpret and assess physician orders to prepare, calculate, and dispense appropriate medication dosages for both inpatient and outpatient care, adhering to the Pharmacy Practice Act and federal regulations.
- Prepare and dispense intravenous admixture solutions using aseptic techniques in a sterile environment.
- Oversee daily operational tasks and assist in supervising pharmacy technicians, aides, interns, and other personnel involved in pharmaceutical care.
- Fill prescription orders in compliance with state and federal laws, utilizing advanced technological resources.
- Provide drug information and educational programs to healthcare professionals and pharmacy students.
- Collaborate with physicians and nursing staff to clarify any unclear or incomplete orders.
- Maintain the formulary by minimizing non-formulary procurements and promoting rational drug therapy selection.
- Adhere to all regulatory requirements and professional standards in delivering pharmaceutical care.
Qualifications:
A Bachelor’s degree in Pharmacy or a Doctor of Pharmacy (Pharm D.) from an accredited institution is required. A minimum of 3-6 months of experience in a hospital pharmacy setting is preferred. Familiarity with investigational drug control procedures and protocol design is also advantageous.
Licensure and Certifications:
