Job Summary

Key Responsibilities
  • Provide accurate assessment, diagnosis, treatment, and management of health problems, as well as health counseling and disposition planning.
  • Use knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care.
  • Counsel and educate patients and families on health maintenance and respond to patient care inquiries.
  • Evaluate and treat patients' medical needs with safe and high-quality treatment.
  • Document all patient care within an electronic health record in accordance with current policies and procedures.
  • Work as a member of the clinical care team providing care and coordination of patients with internal and external colleagues.
Requirements
  • Experience in Family medicine or primary care practice preferred.
  • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
  • Ability to work remotely in a clinical care team culture.
  • Ability to work without direct supervision and practice autonomously.
  • Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
