Job Summary:

As a key member of our pharmacy team, you will play a vital role in ensuring the safe and effective dispensing of medications to our patients. Under the supervision of a licensed pharmacist, you will be responsible for dispensing, compounding, procuring, storing, and distributing pharmacy products. You will also provide pharmaceutical care for hospitalized or ambulatory patients, as assigned, and consult with patients and medical personnel regarding medication therapy.

Essential Responsibilities:
  1. Uphold Kaiser Permanente's policies and procedures, principles of responsibilities, and applicable state, federal, and local laws.
  2. Consider the cognitive, physical, emotional, and chronological maturation process in the delivery of services to patients of all age groups served.
  3. Assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and provide pharmaceutical care needed in accordance with department policy.
  4. Dispense, compound, procure, store, package, and distribute pharmacy products according to laws, regulations, standards, established policies, and procedures, and acceptable professional practices; document responsibility by signing or initialing prescriptions, batch sheets, or other forms according to established policies and procedures.
  5. Interview patients to obtain information regarding medication use, medication allergies, and sensitivities; document the information in the patient's medical record or pharmacy information system when appropriate; advise patients of directions for use, medication storage requirements, importance of compliance, precautions, and warnings for medication therapy; advise patients on the use of related devices and the coordination of medication therapy with diet, according to established policies and procedures.
  6. Substitute for pharmacy personnel as necessary.
  7. Evaluate and resolve, according to established policies and procedures, potential medication therapy problems identified through any and all available sources, including the patient and pharmacy information system.
  8. Confer with medical personnel concerning pharmaceutical care and treatment of patients, related clinical diagnosis, drug combinations, and dosage forms, and other factors that might influence the course of treatment and the activity of medications; suggest changes in medication therapy and/or use as appropriate to assure optimum therapeutic results and cost-effective prescribing.
  9. Participate in assigned education and training programs; conduct educational programs for pharmacy, other medical personnel, and patients.
  10. Participate in quality management programs.
  11. Evaluate and manage the medication therapy of hospitalized or ambulatory patients as assigned to assure optimum therapeutic results and cost-effective prescribing.
  12. Make recommendations to management regarding systems or operational improvements to enhance services provided by the department.
  13. Check equipment used to prepare and dispense medications for proper functioning; report deficiencies to appropriate personnel.
  14. Maintain work area and equipment in an organized and clean condition; maintain a safe, secure, and efficient work environment.
  15. Participate in quality management programs.
  16. Answer and make telephone calls, consistent with assigned duties in accordance with telephone access standards.
  17. Exemplify pharmacy's mission and the organization's vision by contributing to the health and satisfaction of members by providing appropriate medications, information, and professional services in a helpful, caring, courteous, and efficient manner.
  18. Perform other duties as required.

Additional Requirements:
  1. Must be licensed as an Intern Pharmacist by the State of California.
  2. Excellent verbal and written English-speaking communication skills.
  3. Knowledge of Federal and State laws, regulations, and standards regarding the provision of pharmaceutical services.
  4. Ability to extract pertinent information from the patient or other sources and relate it to the patient's medical management plan.
  5. Ability to learn and operate pharmacy computer systems and other equipment.
  6. Requires standing for long periods of time, reading fine print, communicating with co-workers and patients, lifting/transporting 1-50lbs, light to moderate carrying, pulling, pushing, walking, frequent bending, stooping, and reaching.
