Job Summary

Theoria Medical is seeking a skilled Physician Assistant to join our team in providing high-quality, patient-centered care to residents in skilled nursing facilities. As a Physician Assistant, you will be responsible for rounding and managing the clinical care of the population at the assigned SNF, prescribing medications and treatments, and providing comprehensive primary care services.

Responsibilities
  • Rounding daily to manage the clinical care of the population at the assigned SNF
  • Prescribing medications and treatments for residents residing at SNFs
  • Providing high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs
  • Timely documentation of all patient care/encounters in Theoria's proprietary electronic health record (EHR) system, ChartEasyTM
  • Availability for phone/secure messaging consultations with facility nursing staff
Requirements
  • Reliable transportation required
  • Timely attendance is imperative
  • Ability to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation
