Key Responsibilities of the Hospital Care Coordinator:
- Facilitates connections, monitoring, planning, and advocacy for patients to help them achieve and sustain optimal functioning.
- Demonstrates a comprehensive understanding of available resources to ensure quality and cost-effective outcomes.
- Effectively manages and oversees an assigned caseload, tailored to the needs, strengths, abilities, barriers, and preferences of each individual.
- Coordinates discharge planning through thorough assessment, referral, and interdisciplinary collaboration.
- Adapts to shifting priorities in a dynamic work environment.
Position Details:
- Employment Type: Full-time
- Work Schedule: Monday to Friday, 8:30 AM - 5:00 PM
- Weekend Availability: Occasional
- On-Call Requirement: No
Required Certifications and Licenses:
- Registered Nurse (RN) license is mandatory.
- Preferred certifications include ACM (Case Management) and CCM (Case Manager).
Educational Qualifications:
- Associates degree in Nursing is required.
- Bachelor's degree in Nursing is preferred.
Experience Requirements:
- A minimum of 2 years of experience in Acute Care is required.
- At least 2 years of experience as a Registered Nurse is required.
