About the Role

This is an exciting opportunity for a dedicated and compassionate professional to work with patients who require specialized care. As a Clinical Case Manager, you will play a crucial role in coordinating patient care, ensuring timely and effective treatment plans, and promoting quality outcomes.

Responsibilities
  • Develop and implement comprehensive care plans that address the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless transitions between care settings.
  • Conduct thorough assessments to identify social determinants of health and provide targeted interventions.
  • Analyze clinical data to inform treatment decisions and optimize patient outcomes.
  • Communicate effectively with patients, families, and interdisciplinary teams to ensure shared understanding and goals.
Requirements
  • Current licensure in Social Work or a related field (RN preferred).
  • Previous experience in Case Management and Discharge Planning.
  • CCM Certification preferred.
