Job Title: Clinical Pharmacist-Solid Organ Transplant

At Northwestern Medicine, we are seeking a highly skilled Clinical Pharmacist-Solid Organ Transplant to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the safe and effective use of medications for our patients undergoing solid organ transplants.

Responsibilities:
  • Interpret medication orders to ensure safety and proper use of medications, identifying potential interactions, allergies, and administration errors.
  • Coordinate with pharmacy, nursing, and prescribers to resolve medication-related problems, demonstrating initiative and sound judgment.
  • Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions, intervening when necessary.
  • Evaluate and document the appropriateness of medication selections for achieving desired therapeutic outcomes.
  • Communicate potential problems and solutions to prescribers, acting as a consultant in pharmacokinetics and adverse reactions.
  • Participate in therapeutic drug monitoring and intervention programs, adjusting therapy as needed.
  • Process medication order information into the Electronic Health Record, ensuring accuracy of records.
  • Maintain comprehensive pharmacy patient profiles, including age, maturation, weight, allergies, diagnosis, and pharmacokinetic parameters.
  • Oversee packaging, labeling, and distribution of medications, monitoring controlled inventories and resolving discrepancies.
  • Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
  • Verify medication orders, ensuring accuracy and completeness, and resolve discrepancies as necessary.
  • Review laboratory data for appropriateness of medication use, recommending changes when needed.
  • Develop and present education and training programs for medical, nursing, and pharmacy staff, as well as assist in educating pharmacy students, residents, and fellows.
  • Educate patients and their families about specific medications, responding to emergency situations and providing accurate and timely medication information.
  • Maintain the hospital formulary, limiting medication usage to formulary items when appropriate, and document all clinical activities and interventions.
  • Collect data, maintain logs, and record other documentation as required by the department.
  • Communicate professional knowledge effectively in both oral and written forms to patients and other health professionals.
  • Participate in quality improvement and medication use review activities of the department.
  • Organize, prioritize, and plan workload within given time constraints, working cooperatively with fellow pharmacists to oversee and direct the activities of the work area.
Qualifications:
  • Current license to practice pharmacy in the State of Illinois.
  • Bachelor's degree in Pharmacy or a Doctor of Pharmacy Degree.
  • Preferred: Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.), completion of a pharmacy residency program or equivalent hospital pharmacy experience.
