Job Title: Clinical Pharmacist-Solid Organ Transplant

At Northwestern Medicine, we are seeking a highly skilled Clinical Pharmacist-Solid Organ Transplant to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the safe and effective use of medications in our patients undergoing solid organ transplantation.

Responsibilities:
  • Interpret medication orders to ensure safety and proper use of medications, and identify potential medication administration errors.
  • Coordinate the solving of medication-related problems between pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
  • Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions, and intervene as necessary.
  • Evaluate and document the appropriateness of medication therapy, and communicate potential problems and solutions to prescribers.
  • Act as a consultant in pharmacokinetics and adverse reactions to physicians, nurses, and other healthcare professionals.
  • Participate in therapeutic drug monitoring or intervention programs, adjusting therapy as indicated by patient condition and laboratory results.
  • Process medication order information into the Electronic Health Record to ensure accuracy of records.
  • Maintain comprehensive pharmacy patient profiles, including age, maturation, weight, allergies, diagnosis, and estimated and actual pharmacokinetic parameters.
  • Oversee the packaging, labeling, and distribution of medications for all patients and employees, according to established policies and procedures.
  • Monitor the use of scheduled medications on nursing units, and maintain controlled inventories of medication in dispensing areas.
  • Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
  • Verify medication orders before delivery to patient care areas, and ensure accuracy and completeness.
  • Resolve discrepancies as necessary, and review laboratory data for appropriateness of medication use.
  • Develop and present orientation, education, and training programs for medical, nursing, hospital, and pharmacy staff, as well as assist in educating pharmacy students, residents, and fellows.
  • Educate patients and their families about specific medications, and respond to emergency situations to assist with medication management.
  • Provide accurate and timely medication information and teaching to healthcare professionals and patients.
  • Maintain the hospital formulary, and limit medication usage to formulary items when appropriate.
  • Document all clinical activities and interventions performed, and collect data, maintain logs, records, and other documentation as required by the department.
  • Communicate professional knowledge effectively in both oral and written forms to patients and other health professionals.
  • Participate in quality improvement and medication use review activities of the department.
  • Organize, prioritize, and plan workload within given time constraints, and work cooperatively with fellow pharmacists to oversee and direct the activities of the work area.
  • Review restocking automated dispensing cabinets throughout the hospital, and resolve any problems.
  • Ensure compliance with hazardous waste collection and disposal procedures, and maintain proficiency and expertise in areas of practice through continuous professional development.
Qualifications:
  • Current license to practice pharmacy in the State of Illinois.
  • Bachelor's degree in Pharmacy or a Doctor of Pharmacy Degree.
  • Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.) is preferred.
  • Completion of a pharmacy residency program or equivalent hospital pharmacy experience is preferred.
