Nurse Practitioner

We are seeking a skilled Registered Nurse (RN) to join our team as a Home Health Care Provider. As a dedicated member of our healthcare team, you will be responsible for delivering high-quality patient care in a home setting.

• Conduct thorough assessments of patients' physical and emotional needs.
• Develop and implement personalized care plans tailored to each patient's unique requirements.
• Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
• Collaborate with interdisciplinary teams to ensure seamless care coordination.
• Communicate effectively with patients, families, and healthcare professionals to ensure informed decision-making.

• A current license as a Registered Nurse in Delaware.
• A minimum of one year of recent, verifiable clinical experience.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal and communication skills.

• A competitive estimated salary range of $65,000 - $90,000 per year, based on experience.
