About the Role

Aveanna Healthcare is seeking a compassionate and skilled Licensed Vocational Nurse (LPN) to provide high-quality patient care in the home setting.

Key Responsibilities
  • Provide and document skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient.
  • Deliver and coordinate quality patient care in compliance with physician orders, ensuring the highest level of patient satisfaction and outcomes.
  • Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner, maintaining accurate and detailed records.
  • Participate in and update the nursing care plan, taking into account patient needs and responses, and implementing care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability, adhering to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both staff and patient, in accordance with OSHA standards.
  • Educate patients and families regarding disease processes, self-care techniques, and prevention strategies, ensuring they meet their nursing needs.
  • Maintain knowledge of competencies related to the nursing profession, participating in educational programs, continued education units, internal learning management skills, and skill evaluations.
Requirements
  • Graduate of an accredited school of nursing.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One (1) year of experience required working under current nursing license, with six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience preferred.
  • Ability to speak, write, read, and understand English.
  • Ability to travel, reposition patients, and move equipment without assistance.
  • Visual and hearing acuity required.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
