Position Overview:
This role is eligible for the Education Debt Reduction Program (EDRP), a reimbursement initiative for student loan payments.

To qualify, candidates must fulfill specific individual eligibility criteria as per VHA policy and submit their EDRP application within four months of their appointment.

Key Responsibilities:
  • As a Licensed Practical Nurse (LPN), you will serve as an advocate for patients and their families, ensuring safety by operating within your scope of practice and adhering to local policies and procedures.

It is important to note that the scope of practice for LPNs does not encompass medical diagnosis or the prescription of therapeutic or corrective measures.

The practice of vocational nursing must occur under the supervision of a Registered Nurse (RN), Advanced Practice Registered Nurse (APRN), physician, physician assistant, podiatrist, or dentist.

In the primary care clinics, the LPN will oversee daily operations in support of a primary care provider, which includes:

- Conducting patient screenings, including vital signs and documenting the reasons for seeking care.
- Completing clinical reminders and administering medications via oral, intramuscular, and topical routes.
- Providing immunizations, making patient calls, and managing follow-up messages.
- Assisting with dressing changes and supporting providers during invasive procedures such as joint injections.

Each assigned LPN will perform blood pressure and glucose checks as directed by the provider staff. All clinical documentation will be recorded in the electronic patient record, adhering to strict security protocols.

Work Schedule:
Monday to Friday, 8:00 AM to 4:45 PM.

Education Debt Reduction Program (EDRP):
This position qualifies for EDRP, a student loan reimbursement program.

Citizenship:

Education:
Graduate of a practical or vocational nursing program approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time of completion.
Licensure:

Must possess full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.

Proficiency in both spoken and written English is required for licensed practical or vocational nurses in direct patient care roles as mandated by 38 U.S.C. 7402(d) and 7407(d).

May qualify based on the Grandfathering Provision as outlined in the VA Qualification Standard for this occupation (applicable only to current VHA employees in this role who meet the criteria).


Grade Determinations:
GS-3: None beyond the basic requirements.

GS-4: Candidates may be appointed at or advanced to GS-4 if they meet any of the criteria in paragraphs 3b(2)(a) through (c) below: (a) Six months of qualifying experience as an LPN or LVN; or (b) Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience involving nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility providing practical knowledge of human body structure and sterile techniques; or (c) Graduation from an approved school of at least 24 months duration.

(d) LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level but are expected to exercise greater judgment, require less supervision, and operate in accordance with established policies, procedures, and techniques.

GS-5 (a) Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

(a) Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with various physical and/or behavioral issues.

(b) Demonstrated ability to serve as a responsible member of the nursing team and interact appropriately with patients, family members, and other professionals involved in patient care, incorporating acceptable customer service standards.

(c) Knowledge and skill sufficient to prepare, administer, and document actions related to commonly prescribed medications as permitted by local facility policies and procedures.

(d) Ability to recognize the need for and institute emergency measures when necessary, promptly seeking assistance from the RN or MD/DO and assisting in resuscitation procedures.

(e) Recognizes and appropriately responds to equipment malfunctions, safety hazards, and supply deficiencies, promptly reporting to the appropriate personnel.

(f) Completed work is under the general supervision of an RN or MD/DO. Individuals at this grade level are expected to have a broad working knowledge of practical nursing procedures, although more complex practices may require closer review.

(a) Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.

(b) Technically proficient in initiating, performing, and completing assigned duties in providing care to diverse patient populations.

(c) Knowledge and ability to appropriately carry out assigned patient care based on patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report, and record relevant patient information.

Completed work should require only a general review by an RN or MD/DO for appropriateness and conformity with established policies/procedures.

(d) Ability to observe, identify, and respond to the patient's needs for care, including medication, equipment-assisted care, and patient/family education. In organizing and delivering care, the LPN/LVN considers emotional, cultural, spiritual, socio-economic, and age-related factors. (e) Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and performs treatments according to established policies/procedures.

Observes for physical and/or emotional changes in the patient's condition.
Preferred Experience:
1. Two years of experience as an LVN.
2. Proficiency in computer skills, including Microsoft Word, Excel, and navigating a computerized patient record system.
3. At least two years of experience in a Primary and Ambulatory Care setting is preferred.
