Nurse Care Specialist

We are seeking a qualified Registered Nurse to provide direct patient home health visits. Our goal is to help patients obtain a faster recovery, regain independence, and become as self-sufficient as possible to prevent rehospitalizations.
Key Responsibilities:
Provide skilled nursing care to patients in their homes
Conduct thorough assessments and develop individualized care plans
Collaborate with healthcare teams to ensure seamless care transitions
Maintain accurate records and communicate effectively with patients and families
Requirements:
Current, valid, and unencumbered Tennessee RN license or Compact license
Current and valid driver license, transportation, and auto insurance
One or more years clinical experience
Self-motivated and customer service oriented
