Job Title: LPN/RN Private Duty Nurse

We are seeking a skilled LPN/RN Private Duty Nurse to join our team at Phoenix Home Care & Hospice in Sullivan, MO.

Job Summary:

The LPN/RN Private Duty Nurse will provide high-quality nursing care to clients in their homes, focusing on their unique needs and goals. This role requires strong communication and problem-solving skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Develop, review, and revise client care plans in collaboration with the healthcare team.
  • Provide nursing care in accordance with physician orders and individual client plans.
  • Observe, record, and report client reactions to treatment and any changes in condition to the healthcare team.
  • Provide ongoing instruction and supervision to clients, families, or other healthcare personnel as needed.
  • Monitor and record client condition, documenting provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, and staff meetings as required.
Requirements:
  • Active LPN/RN license in the state of Missouri.
  • Problem-solving skills and ability to multitask.
  • Excellent communication skills.
  • Positive, professional attitude.
