About Us
We are looking for a skilled Caregiver Specialist to join our team. As a Caregiver Specialist, you will be responsible for providing patient-centered, evidence-based nursing care activities to meet the physical and psychosocial needs of patients. You will work collaboratively with the care team to ensure quality, cost-effective safe patient care practices.
Your Responsibilities
- You will perform delegated patient care activities using evidence-based practice and specific competencies for the patient population served.
- You will assist in meeting the care needs of the patient in collaboration with the care team to ensure quality, cost-effective safe patient care practices.
- You will demonstrate clinical expertise in the provision of care in the clinical specialty assigned and perform all functions of the Certified Nursing Assistant (CNA), which are age appropriate, developmentally sensitive, and culturally specific.
- You will respectfully address situations or decisions to ensure patient concerns and needs are met.
- You will identify ways to improve the patient's experience of care, streamline care processes, and lower costs to improve patient, family, and team member satisfaction.
- You will identify and escalate patient risk to prevent crises.
- You will demonstrate professional and therapeutic relationships with patients, families, team members, and providers.
- You will reinforce patient education to improve care outcomes and safe patient practices.
- You will perform and document patient cares including personal hygiene, mobility, activities of daily living, and vital signs, and ensure the care environment promotes patient safety.
- You will provide care to patients including collecting body fluids for testing and cultures, administering medications if applicable, and providing patient care as defined within the nursing plan of care and/or physician orders, within the scope of the CNA role.
Qualifications
- You must have a High school diploma or GED.
- You must have Valid State of Wisconsin CNA Certification.
- You must have American Heart Association Basic Life Support (BLS) or CPR certification for healthcare providers.
Salary Information
The estimated annual salary for this position is $63,000-$80,000 depending on qualifications and experience.
