Key Responsibilities

  • Collaborate with the Registered Nurse to implement patient-specific care plans.

  • Deliver high-quality skilled nursing care to patients.
  • Maintain thorough documentation of care provided in our electronic medical records system.

Qualifications

To be considered for this role, candidates must possess:

  • A current Vocational/Practical Nurse License.
  • A valid state driver's license and auto liability insurance.
  • A minimum of one year of experience as a Licensed Vocational/Practical Nurse in a clinical setting, with home health experience preferred.
