We are seeking a skilled Physical Therapist to join our dynamic team at Atlantic Visiting Nurse Dept in Union County. As a Per Diem Physical Therapist, you will have the opportunity to work with patients in their homes, providing comprehensive care and support.
Key Responsibilities:- Evaluate and treat patients in their homes
- Develop and implement individualized treatment plans
- Collaborate with patients, families, and healthcare teams to ensure seamless care
- Minimum Education: High School Diploma or GED, required
- Minimum Experience: 1 year of experience in homecare, strongly preferred
- Licenses/Certifications: NJ License in Physical Therapy, BLS preferred
