We are seeking a skilled Physical Therapist to join our dynamic team at Atlantic Visiting Nurse Dept in Union County. As a Per Diem Physical Therapist, you will have the opportunity to work with patients in their homes, providing comprehensive care and support.
Key Responsibilities- Evaluate and treat patients in their homes
- Develop and implement individualized treatment plans
- Collaborate with patients, families, and healthcare teams to ensure seamless care
- Provide education and support to patients and families
- High School Diploma or GED, required
- Bachelor's in Physical Therapy, required
- Master or Doctorate in Physical Therapy, preferred
- 1 year of experience in homecare, strongly preferred
- NJ License in Physical Therapy, required
- BLS, preferred
