Care Manager RN

We are seeking a skilled Care Manager RN to join our team at Santa Rosa Memorial Hospital. As a key member of our care management team, you will play a vital role in ensuring the best possible outcomes for our patients.
Key Responsibilities- Manage specific patients throughout their admission to ensure the availability of sufficient clinical and medical center resources.
- Communicate daily with medical staff, medical center personnel, and outside resources to optimize resource utilization according to clinical and financial standards.
- Monitor medical records for variances to clinical and financial standards and intervene to achieve optimal patient and medical center outcomes.
- Apply risk management and infection control knowledge to support the healthcare team in achieving desired outcomes.
- Identify issues that expose the organization to liability and financial risk, and collaborate with management, medical staff, and medical center personnel to minimize risk.
- Provide payor/utilization review organizations with concurrent retrospective utilization review required for reimbursement.
- Bachelor's Degree in Nursing.
- Coursework/Training - Environmental Orientation due within 30 days of hire.
- Coursework/Training - Case Management Competency due within 90 days of hire.
- Coursework/Training - General Nurse Competency due within 30 days of hire.
- California Registered Nurse License upon hire.
- 3 years Acute nursing experience, with evidence of progressive clinical experience, or Case management experience.
- CCM upon hire.
