About the Role

We are seeking a skilled Care Manager RN to join our team at Providence Medical Foundation. As a Care Manager RN, you will play a vital role in delivering high-quality, patient-centered care to our patients.

Key Responsibilities
  • Collaborate with patients, nurses, social workers, physicians, and other healthcare professionals to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' needs and develop strategies to address their physical, emotional, and social needs.
  • Coordinate with healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
  • Provide education and support to patients and their families to promote health literacy and self-management.
  • Participate in quality improvement initiatives to enhance the delivery of care and patient satisfaction.
Requirements
  • Graduate of an accredited school of nursing.
  • California Registered Nurse License upon hire.
  • 3 years of Clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.
Preferred Qualifications
  • Bachelor's Degree in Nursing or Master's Degree in Related field.
  • Certification in Case Management (CCM, ACM) upon hire.
  • Specialty Certification in Oncology, Rehab, Pediatrics, Transplant, or Wound Care upon hire.
  • 3 years of Experience in utilization management or case management.
