Licensed Vocational Nurse

Job Summary:
We are seeking a qualified Licensed Vocational Nurse (LVN) to provide health assessments and home health visits for our patients. As a key member of our team, you will be responsible for conducting home health visits independently, obtaining and documenting medical histories, and administering immunizations.
Responsibilities:
- Conduct home health visits independently and provide health assessments to patients
- Obtain and document medical histories, including patient consent and immunization records
- Administer immunizations and medications as prescribed by physicians
- Complete documentation of assessment and/or interventions of patient
- Responsible for home health visits and minor troubleshooting of test systems
- Identify problems that may adversely affect test performance or reporting of test results and take appropriate action
- Perform all relevant diagnostic tests as required
- Perform venipuncture as required
- Complete accepted medical forms
- Conduct weekly equipment check in exam rooms and report any equipment malfunctions
- Code Medical exams as needed
- Administer vaccines
- Administer medication to patients and measure vital signs
- Monitor the patient's progress and changes in behavior
- Recognize and treat any medical condition with appropriate techniques, equipment, and knowledge of all State and County protocols
- Recognize situations which require the immediate attention of a physician or emergency care
- Interact with patients and their families in a compassionate manner
- Interact with clinicians, other health professionals and staff in a courteous and professional manner
- Effectively communicate and interact with patients and/or agencies
- Participate in ongoing professional development activities
- Maintain a positive and professional work environment
- Maintain the confidentiality of all Personal Health Information as dictated by HIPAA laws
- Other duties as assigned
Requirements:
- LVN License
- Blood Withdrawal Certification
- Phlebotomy experience required
- A minimum of 6 months of LVN experience
- BLS with American Heart Association
- Valid CA Driver's License (minimum of 2 years) and acceptable driving record
- Excellent verbal communication skills
- Ability to lift lbs
- Patient bedside experience preferred
- Spanish speaking preferred
- Must be comfortable driving and making home health visits independently
