Job Title: Licensed Vocational Nurse

Job Summary:
We are seeking a qualified Licensed Vocational Nurse (LVN) to provide health assessments and home health visits for our patients. As a key member of our team, you will be responsible for conducting home health visits independently, obtaining and documenting medical histories, and administering immunizations.

Responsibilities:

  • Conduct home health visits independently and provide health assessments to patients
  • Obtain and document medical histories, including patient consent and immunization records
  • Administer immunizations and medications as prescribed by physicians
  • Complete documentation of assessment and/or interventions of patient
  • Responsible for home health visits and minor troubleshooting of test systems
  • Identify problems that may adversely affect test performance or reporting of test results and take appropriate action
  • Perform all relevant diagnostic tests as required
  • Perform venipuncture as required
  • Complete accepted medical forms
  • Conduct weekly equipment check in exam rooms and report any equipment malfunctions
  • Code Medical exams as needed
  • Administer vaccines
  • Administer medication to patients and measure vital signs
  • Monitor the patient's progress and changes in behavior
  • Recognize and treat any medical condition with appropriate techniques, equipment, and knowledge of all State and County protocols
  • Recognize situations which require the immediate attention of a physician or emergency care
  • Interact with patients and their families in a compassionate manner
  • Interact with clinicians, other health professionals and staff in a courteous and professional manner
  • Effectively communicate and interact with patients and/or agencies
  • Participate in ongoing professional development activities
  • Maintain a positive and professional work environment
  • Maintain the confidentiality of all Personal Health Information as dictated by HIPAA laws
  • Other duties as assigned


Requirements:

  • LVN License
  • Blood Withdrawal Certification
  • Phlebotomy experience required
  • A minimum of 6 months of LVN experience
  • BLS with American Heart Association
  • Valid CA Driver's License (minimum of 2 years) and acceptable driving record
  • Excellent verbal communication skills
  • Ability to lift lbs
  • Patient bedside experience preferred
  • Spanish speaking preferred
  • Must be comfortable driving and making home health visits independently
