Job Summary:

Nuvance Health is seeking a skilled Speech Therapist to join our Home Health team. As a Speech Therapist, you will work closely with patients, families, and healthcare professionals to provide high-quality speech therapy services in the comfort of patients' own homes.

Responsibilities:

  • Assess patients' speech and language needs, develop individualized treatment plans, and provide evidence-based therapy to improve communication skills.
  • Collaborate with healthcare teams to coordinate care, ensure seamless transitions, and achieve optimal patient outcomes.
  • Document patient progress, maintain accurate records, and communicate effectively with patients, families, and healthcare professionals.
  • Participate in case management conferences, quality assurance activities, and performance improvement initiatives to ensure high-quality patient care.
  • Supervise home health aides as needed, provide education and training to patients and families, and promote a culture of excellence within the Home Health team.

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current licensure to practice as a Speech-Language Pathologist in the State of Connecticut.
  • Successful completion of the National Registration Examination (NRE) or equivalent.
  • Minimum one year of experience in speech therapy, preferably in a home health or rehabilitation setting.
  • Functional Independent Measurement (FIM) certification and CPR certification.
  • Experience with FEES or MBS is a plus.
