About the Position

We are seeking a dedicated Client Care Coordinator to join our team, providing vital support to clients and their families in achieving healthy relationships, emotional stability, personal development, and adjustment.

The ideal candidate will possess excellent counseling skills, working closely with clients to address complex issues, including abusive behavior, family dynamics, healthy personal choices, substance abuse, mental and physical health/trauma related concerns.

Main Responsibilities
  • Collaborate with clients to develop and implement individualized counseling plans, focusing on healthy relationships and emotional stability.
  • Conduct comprehensive risk assessments, identifying potential areas of concern and recommending strategies for mitigation.
  • Maintain accurate biopsychosocial evaluations, adhering to regulatory requirements.
  • Respond promptly and effectively to crisis situations, ensuring clients' immediate well-being and safety.
Compensation

The salary for this position is expected to range from $58,000 to $65,000 annually, depending on qualifications and experience.

Requirements
  • A Master of Social Work degree is required; a Licensed Master of Social Work degree within one year of hire is highly desirable.
  • Candidates must undergo background checks, obtaining clearances through the State Central Registry, The Register of Substantiated Category One Cases of Abuse or Neglect, and the Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services.
