Job Title: Case Manager

Job Summary:

We are seeking a skilled Case Manager to join our team at Horizon Services, Inc. The successful candidate will provide case management services to clients residing at our Detoxification Center, coordinating their withdrawal management needs and ensuring their basic needs are met in preparation for stabilization and discharge planning.

Key Responsibilities:

  • Provide case management services to clients, including care coordination, scheduling appointments, and arranging transportation.
  • Assist Detoxification Department staff with coordinating client withdrawal management needs.
  • Schedule and arrange client medical, psychiatric, and continuing care appointments.
  • Provide ASAM screening and referral assessments as needed.
  • Maintain accurate documentation in accordance with Case Management billing practices.
  • Provide timely crisis intervention when needed.
  • Meet daily with clients to determine case management and referral needs.
  • Participate in all required consultation with San Mateo County regarding ASAM and other requirements.
  • Maintain positive working relationships with community providers and managed care organizations.
  • Assist with case management training at monthly staff meetings.
  • Provide clients with educational materials and orientation of continuing care options.

Requirements:

  • Knowledge of alcohol and drug addiction.
  • Excellent oral and written communication skills.
  • Excellent computer skills, knowledge of data capture and data input into Electronic Health Records.
  • Ability to communicate using email and other electronic systems.
  • Familiarity with Substance Use programs, hospitals, and other continuing care options.
  • Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention).
  • Experience working with clients who have Substance Use disorders.
  • Extensive experience with case management of underserved populations in a public setting.
  • Experience working with diverse populations and cultures.
  • Experience working with customers or clients with co-occurring disorders and disabilities.
  • Familiar with customer service principles.
  • Experience with resolution of conflict in stressful situations.

Preferred Qualifications:

  • Registered or certified with a State AOD certifying body such as CCAPP, CADTP, or CAADE.
  • Valid California driver's license with a good driving record and car insurance.
  • Current CPR/First Aid certificate desirable.

Salary: $28.00-$30.00/hr based on experience and qualifications. Available for a full-time position.
