Position Overview & Responsibilities:
As a Licensed Practical Nurse, you will provide direct nursing care to residents while overseeing the care delivered by medication care specialists. Responsibilities include administering medications, performing treatments, obtaining specimens, and maintaining accurate documentation as required.

The essential duties outlined below represent the core functions of this role.

  • Document resident conditions in accordance with policy.
  • Supervise assigned team members while providing direct nursing care to residents.
  • Assist with the onboarding of new residents.
  • Support residents with Activities of Daily Living (ADL) and encourage an independent lifestyle.
  • Respond promptly and courteously to resident calls.
  • Identify changes in resident health and behavior, developing and monitoring care plans accordingly.
  • Provide daily updates to the Community Wellness Director regarding community conditions and nursing supply needs.
  • Deliver nursing care in alignment with physician orders and established nursing practices.
  • Ensure residents receive preventative, therapeutic, and rehabilitative services, coordinating care with other disciplines.
  • Accompany physicians during rounds, assisting in examinations and treatments.
  • Assess resident conditions and notify the attending physician and responsible parties of any changes.
  • Administer prescribed medications accurately and supervise the continuity of nursing care.
  • Lead and mentor assigned team members, demonstrating effective decision-making skills.
  • Evaluate and monitor complex resident health statuses, adjusting care plans as necessary.
  • Orient residents and team members to emergency protocols and respond to emergencies as needed.
  • Complete incident reports and follow up as required.
  • Ensure all resident records are accurate and complete, maintaining compliance with regulatory standards.
  • Monitor the resident environment to uphold safety and sanitary regulations.
  • Embrace a culture of hospitality, practicing excellent customer service in all interactions.
  • Uphold Residents' Rights policies consistently.
  • Adhere to all community policies and procedures.
  • Accept assigned duties and instructions cooperatively, addressing concerns professionally.
  • Perform incidental housekeeping and maintenance tasks to maintain a clean and safe environment.
  • Carry out all other related duties in a timely and professional manner.

Qualifications:

  • Current nursing license in the state of employment.
  • Minimum one year of nursing experience, with preference for experience with Alzheimer's or dementia patients.
  • Strong interpersonal and teamwork skills, with the ability to process written information effectively.
  • Demonstrates integrity and compassion towards cognitively impaired older adults.
  • Required to receive the annual Flu vaccine, barring religious or medical exemptions.
