We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our HouseCalls team in North Carolina. This is a part-time opportunity, with the flexibility to work 24 hours per week.

As a member of our integrated medical teams, you will provide annual clinical assessments to patients in their homes, helping identify and reduce health risks while coordinating follow-up care to improve overall well-being.

About the Role

You will conduct in-home assessments, including evaluating past medical history, reviewing systems, medication reconciliation, vital signs, and comprehensive physical exams. You will also perform evidence-based practice screenings, identify diagnoses, formulate a list of current and past medical conditions, communicate findings with primary care physicians, and intervene in urgent situations.

In addition to your clinical expertise, you will educate patients on disease processes, medication, and compliance. This role requires strong communication skills, empathy, and a patient-centered approach.

Requirements
  • A valid NP or PA license, with certification from a recognized national organization (ANCC or AANP for NPs, NCCPA for PAs).
  • The ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs.
  • Reliable transportation for home visits.
