Job Summary:

We are seeking a skilled Acute Transition Care Manager to join our team at Vaya Health. As a key member of our care management department, you will be responsible for providing proactive intervention and coordination of care to our members who are receiving care in an inpatient community hospital or emergency department.

About the Role:

This is an exciting opportunity for a registered nurse with experience in acute care management to work with our multidisciplinary care teams to ensure seamless transitions of care. You will utilize your clinical expertise to assess needs, develop transition plans, and provide education and support to members and their caregivers.

Responsibilities:
  • Conduct thorough assessments of members' needs and develop comprehensive transition plans
  • Work collaboratively with multidisciplinary care teams to ensure seamless transitions of care
  • Provide education and support to members and their caregivers to promote self-management and independence
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
  • Active RN license in North Carolina
  • At least 2 years of experience in acute care management
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
