We are seeking a compassionate and skilled Hospice Registered Nurse Case Manager to join our team at Interim HealthCare of Sacramento. As a Hospice Registered Nurse Case Manager, you will play a vital role in providing end-of-life care to patients and their families.
Key Responsibilities:- Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs to develop individualized care plans.
- Coordinate interdisciplinary care team meetings to ensure collaboration among healthcare professionals involved in the patient's care.
- Educate patients and their families about disease progression, symptom management, and end-of-life care options.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
- Provide compassionate end-of-life care, including symptom management and support for patients and their families.
- Graduate of an accredited nursing program and active RN license in California.
- Minimum of 1 year of nursing experience in hospice.
- CPR certification.
- Knowledge of state and federal home health regulations.
