Key Responsibilities- Provide case management and milieu management services to patients and families.
- Complete psychosocial assessments and assist in the development and re-evaluation of treatment plans.
- Work collaboratively with a multidisciplinary team to coordinate discharge planning.
- Maintain accurate and timely documentation of patient records.
To be successful in this role, you will need:
- A Master's Degree in Social Work or a related field.
- Current licensure as a Social Worker in the state of Michigan.
- At least one year of experience in a hospital or outpatient setting, with a focus on mental health care.
- Excellent communication and interpersonal skills.
