Job Overview

We are seeking a skilled Nurse Specialist to provide direct nursing services and case management for patients in home health care and infusion therapy settings. This role requires collaboration with patients, caregivers, physicians, and other healthcare professionals to deliver high-quality patient care.

Salary and Benefits

The estimated annual salary for this position is $85,000-$110,000 based on experience and location. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Our comprehensive benefits package includes a 401(k) retirement plan, health insurance, life insurance, disability insurance, time off benefits, and much more.

Job Responsibilities

As a Nurse Specialist, you will:

  • Provide direct nursing services to patients in their homes, clinics, or alternate living sites.
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans.
  • Manage infusion therapy services, including administration, education, and monitoring of patients.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Requirements and Qualifications

To be successful in this role, you will need:

  • A minimum of one year of experience in acute care nursing.
  • Experience in home health or home infusion care is highly desirable.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Proficiency in electronic medical records and other healthcare technologies.
