Job Title: Nurse Navigator - Surgical Oncology

Northwestern Memorial Healthcare is seeking a skilled and compassionate Nurse Navigator to join our Surgical Oncology team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between departments.

Job Summary:

The Nurse Navigator will work closely with patients, families, and healthcare providers to provide comprehensive care and support. This includes assessing patient needs, developing individualized care plans, and coordinating appointments and treatments. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Collaborate with patients, families, and healthcare providers to develop and implement individualized care plans.
  • Assess patient needs and identify opportunities for care coordination and support.
  • Coordinate appointments, treatments, and follow-up care to ensure seamless transitions between departments.
  • Provide education and support to patients and families on diagnosis, treatment options, and self-care.
  • Communicate effectively with healthcare providers and other team members to ensure timely and effective care.
  • Identify and address potential barriers to care, including social determinants and healthcare disparities.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Qualifications:
  • Current RN licensure in the state of Illinois.
  • Minimum 2 years of experience in a healthcare setting, preferably in oncology or surgical services.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Basic computer knowledge and proficiency in electronic medical records.
