Job Summary

Under the supervision of a Director/Manager/Supervisor, this role performs health information activities necessary to organize, maintain, and use electronic and paper patient health records. The incumbent may specialize in one or more technical functions, including record maintenance, indexing, scanning, and validation, electronic and physical chart retrieval, and reconciling medical record activity. The role also involves accessing and updating electronic tracking systems, answering telephones, and assisting with continuity of care requests for medical records and the patient portal.

Key Responsibilities
  • Maintains accurate and up-to-date patient health records in accordance with established policies and procedures.
  • Indexes, scans, and validates medical records to ensure accuracy and completeness.
  • Retrieves electronic and physical charts as needed to support patient care and medical research.
  • Reconciles medical record activity to ensure accuracy and completeness.
  • Accesses and updates electronic tracking systems to support patient care and medical research.
  • Provides excellent customer service to patients, families, and healthcare providers.
  • Assists with continuity of care requests for medical records and the patient portal.
Qualifications
  • High School Diploma or G.E.D.
  • One (1) year of customer service experience.
  • Knowledge and competencies in Health Information Practices and Technologies, including medical terminology.
  • Applied analytical skills.
  • Advanced problem-solving skills.
  • Wide-range attention to detail.
  • Strong professional customer service and communication skills.
  • Proficiency with basic computer programs and intermediate keyboarding skills.
  • Detailed knowledge of medical record content and regulatory standards for documentation and privacy.
Preferred Qualifications
  • Some college, preferably Health Information Management or Health Information Technology, Informatics or Management degree.
  • Experience in Medical Records or Health Information-related department, preferably hospital.
  • Knowledge of hospital computer systems and electronic medical records, preferably Epic and/or OnBase.
