  • Collaborate with the interdisciplinary team to assess and refine care plans
  • Monitor, document, and communicate resident progress
  • Administer prescribed medications and implement treatment plans
  • Maintain accurate nursing and administrative documentation
  • Participate in staff meetings and professional development programs
Qualifications
  • Successful completion of an accredited Licensed Practical Nurse program and licensure in New York State
