Hospice Registered Nurse Case Manager

We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Hospice Home Care. As a RN Case Manager, you will play a vital role in providing high-quality care to our patients and their families.
Key Responsibilities:- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
- Administer medication and treatment as prescribed by the physician, while also providing emotional and spiritual support to patients and their families.
- Ensure compliance with all state and federal regulations, as well as our organization's policies and procedures.
- Participate in ongoing education and training to stay up-to-date on the latest hospice care practices and philosophies.
- Graduate of an accredited nursing school with current licensure to practice in the state of Arkansas.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
