Hospice Nurse Case Manager

We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Gentiva Hospice. As a Hospice RN Case Manager, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.
Key Responsibilities- Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and coordinating with healthcare teams.
- Act as a liaison between patients, families, and healthcare providers to ensure seamless care transitions.
- Empower patients and their caregivers by educating them on care management and providing emotional support.
- Collaborate with interdisciplinary teams to deliver comprehensive care and address patient needs.
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
- RN license in the state of employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Flexibility and adaptability in a fast-paced environment
- Current driver's license and automobile insurance
