Key Duties:
- Facilitate individual and group counseling sessions utilizing evidence-based techniques.
- Work collaboratively with a multidisciplinary team to create and execute comprehensive treatment strategies.
- Evaluate and track client progress, modifying interventions as necessary.
- Contribute to the success of the treatment center by engaging in team meetings and professional growth activities.
- Develop discharge and continuity of care plans to ensure appropriate transitions to different levels of care.
- Manage a patient caseload, including all case management, treatment planning, and clinical documentation as required.
