We are seeking a skilled Registered Nurse to join our Transitional Care team as a Charge Nurse. In this role, you will be responsible for providing high-quality patient care and leading a team of nurses to achieve exceptional patient outcomes.
Key Responsibilities- Provide direct patient care and develop individualized care plans to meet the unique needs of each patient.
- Lead and manage a team of nurses to ensure seamless care delivery and high-quality patient outcomes.
- Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal results.
- Monitor and report on patient progress, identifying areas for improvement and implementing changes as needed.
- Develop and maintain a culture of safety and quality within the team, promoting a positive and supportive work environment.
- Current Washington Registered Nurse License.
- 3 years of RN experience, preferably in sub-acute care, or previous charge nurse experience, or 2 years relevant leadership experience.
- National Provider BLS - American Heart Association within 30 days of hire.
