Job Title: Registered Nurse Patient Care Manager

LHC Group is seeking a highly skilled Registered Nurse to join our team as a Patient Care Manager in our Home Health division. As a Patient Care Manager, you will be responsible for overseeing the delivery of high-quality patient care and ensuring that our patients receive the best possible outcomes.

Job Summary:
  • Provide leadership and guidance to a team of healthcare professionals to ensure the delivery of high-quality patient care.
  • Develop and implement individualized patient plans of care in collaboration with the interdisciplinary team.
  • Conduct regular patient assessments to identify and address any changes in patient condition or needs.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination.
  • Ensure compliance with all relevant laws, regulations, and company policies.
Responsibilities:
  • Supervise and coordinate the activities of a team of healthcare professionals to ensure the delivery of high-quality patient care.
  • Develop and implement individualized patient plans of care in collaboration with the interdisciplinary team.

  • Develop and maintain relationships with patients, families, and healthcare professionals to ensure a positive patient experience.
Requirements:
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
